Opposition and reposition are two key movements of the thumb that primarily occur at the first carpometacarpal (CMC) joint between the trapezium and the base of the first metacarpal. The lever sign test of the thumb evaluates for carpometacarpal joint arthritis. The examiner stabilizes the thumb and grasps the first metacarpal just distal to the carpometacarpal (basal) joint, then applies a back-and-forth shucking motion in the radial and ulnar directions. A positive test is indicated by reproduction of the patient’s pain localized to the base of the thumb.
