The NASA report is the latest in a series of high-profile setbacks for Boeing. Before the 2024 Starliner mishap, its aviation arm came under fire after a panel blew out midflight on one of its 737 Max 9 airplanes. The 737 Max had previously been grounded after two crashes in 2018 and 2019 killed all onboard.
Jared Isaacman critiqued engineering issues, but said leadership and decision-making was the "most troubling failure" of the Boeing Starliner debacle.
